Introduction

Electronic forms enhance Simploud’s flexibility by adding the ability to attach smart forms to assist with decision making or to capture structured information in an intuitive way. Using electronic forms can be for decision making (e.g., determining if a complaint is reportable or not), checklists for completion, decision trees, scorecards, questionnaires, surveys and other use cases that requires structured form that should be filled by the users.

E-forms are created based on predefined templates that are being created per each object as instructed in this section.

Templates

The template is a foundation of  the electronic form.
The electronic form rests on a template in order to be created, in Simploud there are two types of such templates – Standard and flowchart.

Standard Template

This is the classic template of Simploud, enables to handle vary use cases that requires simple and structured kind of forms. It is simple and user friendly to create and fill.

 

Flowchart Template

Simploud advanced type  template, this is a set of questions that follows a specific logic.  
Every question leads to the next one.
Recommended when a different answer may affect the rest of the template.
 

Creation of a new template

Go to App Launcher  and search for the Templates object, Click on it, at the right – top of the page click on New, then fill in the following required fields:

  • Insert the name of the template.

  • Choose the process to whom the template will be connected.

  • Choose the purpose of the template.
  • Check the Active check-box.

  • Click Save.

 

Just like any other object in Simploud, the creation of a new template creates a new record in the system and that record is being managed via a short workflow. The template’s record page contains 2 sections:

Template settings and Template Questions.

Template Settings

This section is designed to define template features.

In this guide we will deal with two basic features with which you can start using Template comfortably without diving deep into the depths of things.

The features are:

  • This section displays the type of the template.

  • Flowchart :This checkbox determines the type of the template, if checked – flowchart template.

  • Click on Edit to describe the template. Click save when you finish.

  • Click on Save Questions to save changes.

  • Template Definitions

The follow feature permits to add text tab for each question and to show who answered the question.

  • Check Show Comments to add text bar alongside to the question.

  • Check Show Performed By to display the user who answered the question details.

  • Click on Save Questions to save changes.

  • Form Access and Visible Parts

Standard Template Questions

In this section question are added to the template. This paragraph will focus on Standard template questions. Simploud’s flexibility allows you to  define for each question its characteristics simply and comfortably. 

Clicking on Add a Question button displays a pop-up window with the fields to set a new question.

  • ID- Automatically set number to the question

  • Select a Response Type.

  • Insert a relevant question.

 Response Type – Allows to define the data type of the question (further explanation at the picture below).

Flowchart Template Question

As written before, flowchart is an advanced template type. You will find in it smart solutions and tools. The flow chart has specific rules and new question types, such as:  

1) Checkpoint – Setting the next question for each answer.

  • Select CheckPoint as Response Type.

  • Insert answers.

  • Set the next question the user will see and click on Save.

2) Calculated Result – Creating a formula composed of answers of questions (insert question id – the system will take the answer) and arithmetics.

Lorem ipsum, or lipsum as it is sometimes known, is dummy text used in laying out print, graphic or web designs.

3) Signature.

Moreover, flowchart must start with a section type question and finish with a result. The flowchart type is distinct due to the appearance of questions, every question “calls” the next question.
